Issued by the Arizona Corporation Commission (ACC), Corporations Division, not by the Secretary of State. Order online through the Arizona Business Center (ABC) portal or submit Form M048 Records Request Form by post or fax. The base fee is $10 and takes 7-10 business days; paying the $35 expedite, $45 in total, produces the certificate instantly. WHAT IT ACTUALLY CERTIFIES IS NARROWER THAN THE NAME SUGGESTS: under A.R.S. 29-3211 it states that the articles are filed and effective, that no dissolution or termination is on record, and that all fees, taxes, interest and penalties owed TO THE COMMISSION have been paid. It says nothing about Department of Revenue tax standing. A lender or agency wanting proof of tax compliance is asking for the separate Certificate of Compliance, which ADOR issues, not the ACC.
