Issued by the Utah Department of Commerce, Division of Corporations and Commercial Code, not by a Secretary of State. There are two documents at two prices and THEY ARE ORDERED DIFFERENTLY. The Regular, or short form, is $12, is one page, and is bought inside the Business Registration System, which emails it automatically once you pay. The long form is $20, adds the list of filed documents, and CANNOT BE ORDERED ONLINE: send the order form by post to P.O. Box 146705, Salt Lake City UT 84114-6705, hand it in at 160 E. 300 S., 1st Floor, or email it and wait for a payment link. Written orders take 3 to 10 working days. Expedite is $75 per document for 1 to 2 business days, which is pointless on the online short form and is the only way to accelerate the long form. A separate Letter of Good Standing from the State Tax Commission, which is free, addresses tax compliance.
